Sildenafil vs tadalafil: 4 hours vs 36 hours, and what that actually means

Both PDE5 inhibitors. Both effective. But the half-life difference is the whole story — pick the one that matches how you actually want to live.

Sildenafil vs tadalafil

Both medications work the same way: they inhibit phosphodiesterase type 5, which prevents the breakdown of cGMP, which keeps smooth muscle in the penile arteries relaxed, which means more blood flow when you need it. They're equally effective for the underlying job. The difference between them is time.

The half-life difference

Sildenafil (Viagra) has a half-life of about 4 hours. You take it 30-60 minutes before you want it to work, and you have a window of 4-6 hours.

Tadalafil (Cialis) has a half-life of about 17.5 hours. The "weekender" reputation comes from this — a 20mg dose works for 24-36 hours. There's also a daily formulation (2.5 or 5mg) that you take every morning and that essentially keeps your baseline elevated all the time.

Which one for which life

The right choice depends entirely on how you want to use it:

  • You want sex on a schedule (date night, weekend): sildenafil is fine and cheaper.
  • **You want sex not on a schedule** (spontaneous, unplanned, "in the moment"): tadalafil daily eliminates the planning step entirely.
  • You have benign prostatic hyperplasia (urinary symptoms from an enlarged prostate): tadalafil daily is approved for BPH too — one med for two problems.
  • You're sensitive to side effects: sildenafil's shorter duration means side effects (headache, flushing, nasal congestion) clear faster too.

Food and timing

Sildenafil is the picky one. A high-fat meal can delay absorption by an hour or reduce peak concentration by ~30%. Take it on an empty stomach, or at least don't take it after a steak dinner.

Tadalafil doesn't care about food. Eat what you want.

Side effects — the predictable list

Both medications share the same side-effect profile because they have the same mechanism:

  • Headache (most common, 10-15%) — usually mild, often resolves after the first few doses
  • Flushing (5-10%) — feels like a hot flush in the face/chest
  • Nasal congestion (5%)
  • Indigestion (5%)
  • Visual changes with sildenafil specifically (rare; bluish tint to vision)
  • Muscle/back aches with tadalafil specifically (more common with daily dosing)

Drug interaction worth knowing: nitrates (for chest pain) are a hard no with any PDE5 inhibitor. The combination can cause dangerous blood pressure drops.

The honest meta-point

For most patients, daily low-dose tadalafil is the right answer if cost isn't a concern. Removing the "did I take it / when did I take it / will it kick in" cognitive load is a meaningful quality-of-life upgrade. Sildenafil on-demand is the right answer if you want to spend less and don't mind planning.

If neither works at full dose, that's not a failure of the medication — it's a signal to look for underlying causes (vascular disease, low testosterone, sleep apnea, untreated anxiety). The pill is a tool, not a destination.
